FORMS AND LINKS: 

What forms need to be completed and when are they due? 

The following paperwork needs to be turned in BEFORE an athlete can participate in tryouts or practices:

• Athletic Physical -must be dated on April 15 or later of the upcoming season to be good for this school year.  In other words, complete your physical every year AFTER April 15 but before August.  BRING THE FORM to the physical as it needs your doctor’s signature.

• Athletic Code of Conduct

• Concussion Protocol - This only needs to be filled out ONCE during your HS athletic career at FHC.  If you have already done a sport at FHC, we already have one on file.

• Transportation Rules

• Emergency Medical Card

• Participation Form and Fee  - if you are going out for a non-cut sport the $140 participation fee and form is due now, if a cut sport the fee and form is due the first practice after teams are announced.
